Your First Visit

Before you arrive

  • Fill out the Registration Form. Documents may be uploaded here.
  • Stop antihistamines 5 days before your visit.
  • Check your referral requirement. Some plans require one to see a specialist

  • Insurance card and photo ID
  • Referring doctor’s name and contact information
  • Prior records

To save time when making your first visit::

  1. Fill out and upload the Registration Sheet
    and also please register at Dr. Stadtmauer’s Patient Portal
  2. Please bring your insurance card.
  3. Stop antihistamines 3 days prior to your visit (they can interfere with the allergy testing) This includes Alavert, Claritin, Clarinex, Allegra, Xyzal, Zyrtec, Benadryl and others. Do not stop any asthma inhalers.
  4. Do you need a referral to see us? Check with your health insurance company.
  5. Please bring your referring or general doctor’s name and address.
  6. It’s very important for you to understand how health insurance works. Almost all insurance plans require some patient contribution (copay (a fixed amount), coinsurance (a fixed % of the bill), or deductible (your insurance will makes you responsible for the complete bill until you have paid a fixed amount).
